ed210f6b2c more type traits updates:
Header includes get BOOST_ prefix,
BOOST_DECL_MC dropped in favour of new BOOST_STATIC_CONSTANT (from config.hpp),
operator ! dropped in favour of boost::type_traits::ice_not template.
