API Changes:
The signature of basic_parser<>::body_limit(n) has changed. It now accepts an
optional std::uint64_t. The caller may indicate that no body limit is required
by calling body_limit(boost::none). The default limits remain in place in order
to maintain 'safe by default' behaviour.

* `basic_parser` now uses pure virtual member functions instead
of the curiously recurring template pattern.
Actions Required:
* Change uses of the `basic_parser` type to omit the `Derived`
template parameter
* Classes derived from `basic_parser` no longer need to friend
the base.
* Virtual functions in the derived class may be marked `override`.

* BodyReader and BodyWriter constructors now require the header and
body elements to be passed as distinct header and value_type objects.
This enables the composition of body types, for example:

* The previous single-argument constructors are deprecated and will be
removed in a subsequent version.

* Change user-defined instances of BodyReader or BodyWriter constructor
signatures to the two-argument form.

* Define the macro BOOST_BEAST_ALLOW_DEPRECATED in the project (which
will accept both the new and the deprecated signatures).

* upgrade() was is_upgrade()
* keep_alive() was is_keep_alive()
