Add XCode 9.4.1 testing. (#463)

* Add XCode 9.4.1 testing.
Refs https://github.com/boostorg/config/issues/462.

* Disable nullptr and __int128 support on clang/XCode-9.1.

* Correct BOOST_HAS_INT128 undef option.

* Stop testing C++17 mode - it's not supported by the std lib.
