John Spray 66ac7de303 Grant ceph-mgr readonly access to nodes 
We can attempt to get knowledge of nodes from Rook's
configmaps, but going direct is useful for usability in the Ceph UI: users
should be able to distinguish between "node doesn't exist"
and "node exists but Rook is unaware of it".

John Spray cff953696c Set service account on ceph-mgr pod 
This is to enable the `rook` ceph-mgr module to drive
the Rook and k8s APIs, exposing functionality via
its own CLI and GUI.

The service account gets access to two different roles:
one for the items that are within the cluster's namespace,
and one for reading configmaps in the rook-ceph-system namespace,
which is needed for reporting available devices.

John Axel Eriksson dd7860fb8e Support additonal agent mounts and alternate lib/modules host path. 
The background is that there are at least two distributions with very
different host fs layouts. Those would be NixOS and GNU Guix.

For example, on NixOS, /lib/modules does not exist, it can instead
be found at /run/current-system/kernel-modules/lib/modules where
/run/current-system/kernel-modules is a symlink into /nix/store.

Within the modules directory, again on NixOS, there are more links
into the /nix store.

So to support alternate host fs layouts it is now possible to specify
the lib/modules host path and in addition specify extra mounts.

Huamin Chen 82425aafcc prepare osd in a job per node. Once all osds are prepared, store osd info in orchestration configmap. 
Operator watches the configmap, starts one osd replica set per osd.

Alexander Trost edfa135560 ceph mon: Only run one mon on a node 
This introduces a MonSpec to the Ceph Cluster CRD which contains settings for
mons, current options are the count and if more than one mon is allowed
to run per node.

Fixes #1386
