Ceph has recently reported that it may be unsafe to upgrade clusters
from Nautilus/Octopus to Pacific v16.2.0 through v16.2.6. We are
tracking this in Rook issue https://github.com/rook/rook/issues/9185.
Add a warning to the upgrade doc about this.

In Rook v1.8 the min version of K8s supported is updated to 1.16.
Users running on older versions of K8s are recommended to update
to 1.16 or newer before updating to Rook v1.8.

Add a section to the upgrade doc instructing users to (and how to)
migrate `CephRBDMirror` `peers` spec to individual `CephBlockPools`.
Adjust the pending release notes to refer to the upgrade section now,
and clean up a few references in related docs to make sure users don't
miss important documentation.

We should not use .items[0].metadata.name if the array length is 0. This
is the case when nothing has been initialized yet. Instead, we should
use .items[*].metadata.name, the wildcard ensures to always return 0
even if nothing is present yet.

Recently, images moved from docker.io to quay.io so people using main
major tag like `v16` needs to switch to use the quay.io URL.

The section header was updated to Ceph Pacific since the tag used is
`v16.2.5-20210708`, which means Pacific and not Octopus.

Previous release upgrades included instructions for removing obsolete
drive groups and dealing with a preserveUnknownFields error. These
should no longer be an issue with the v1.7 upgrades.

Add sections to docs to mention updating ancillary resources, of which
monitoring is the only one currently.

Recently, the builds of `ceph/ceph` image moved to quay.io, see
https://github.com/ceph/ceph-build/pull/1883 for more details.
Current images will remain but new builds will happen on quay.io only.
This means that tags such as `v14.2`, `v15.2`,`v16.2` will need to
switch to quay.io to get updates.

Ceph v16 will be the default in Rook v1.7. Update references in the
master branch from v15 (currently v15.2.11) to v16 (currently v16.2.4).

This commit updates external-snapshotter version to
v4.0.0 which supports snapshots v1.
Rook now defaults to enabling RBD and CephFS snapshotter
for K8s >= v1.17 and disabling it for K8s <= v1.16.
Supporting changes in documents and examples yaml files
are made.

When upgrading Rook clusters created before v1.5,
`kubectl apply -f crds.yaml` fails due to the existence of
the usupported `spec.preserveUnknownFields`. Add text documenting how to
work around this issue in the Ceph upgrade docs.

Remove features and design that supports adding OSDs to Ceph clusters
via `spec:driveGroups`. Update the Ceph upgrade doc that informs users
who currently use Drive Groups (we believe there are none of these
users) how to migrate to using the `spec:storage` config.

no need to keep testing upgrades from v1.2 all the way to master.
