Update the base operator image and cluster examples to use the latest
octopus release v15.2.4. Also cleanup some old examples and unit tests
that were still based on mimic.

We have introduced a new goroutine to check the state of the rgw
endpoint. It will run every minute and perform operations on a bucket.
The success or failure will be reported as part of the status field of
the CephObjectStore CR.
A good status will look like:
status:
endpointStatus:
lastChanged: "2020-06-25T13:47:45Z"
lastChecked: "2020-06-25T13:48:46Z"
phase: Connected
A failed status:
status:
endpointStatus:
details: |-
error creating bucket "rook-ceph-internal-s3-bucket-checker": RequestError: send request failed
caused by: Put http://rook-ceph-rgw-my-store.rook-ceph:8080/rook-ceph-internal-s3-bucket-checker: dial tcp 10.108.189.148:8080: connect: connection refused
health: ERROR
This check works for both converged and external modes. Note that the
CephObjectStore CRD has a new field called "externalRgwEndpoints" which
allows you to define a list of IP addresses pointing to rgws.

We can now explicitly set any property on a given pool by using the new
Property field in the CephBlockPool Spec.
Also, this fixes the case where both `CephBlockPool` and `CephCluster`
are created at the same time. When Rook creates the pool, the cluster is
still being bootstrapped and the global option
`osd_pool_default_pg_autoscale_mode` has not bee set yet. So the pool
gets created but its `pg_autoscale_mode` property is set to `warn`
instead of `on`.

The volume sources for an OSD on PVC were relying on the ordering
of the volume sources slice. While this was working, it was causing
headaches and potential future developer error when being
interpreted incorrectly. The addition of the wal pvc device
would make the issue even more pronounced. Now each PVC used
for the OSD will be identified in a map by name such as data
and metadata.

This commit contains CR implementations for:
CephObjectRealm
CephObjectZoneGroup
CephObjectZone
Also there are changes made to the objectstore
to add rgws in the object-store to zones and
zone groups in a multisite configuration and
the removal of the --default parameter for any
realms/zonegroups/zones that are created.

When the PVC template increases in size for the storageClassDeviceSet
in the cluster CR, we need to increase the size of the PVC and restart
the OSD. The OSD is restarted by changing a variable on the pod spec
that indicates the desired size of the OSD.

Now, we can not only cleanup monitor data, logs and crashes but the
disks too. As part of the cleanupPolicy CR spec, we have a new setting
called sanitizeDisks which holds more details:
* confirmation: the confirmation message to sanitize disks,
use "yes-really-sanitize-disks" to confirm
This will **only** wipe the metadata, so it's a fast cleanup allowing
you to re-install later but won't remove all the data from the drive.

The lack of the json tag required manifests to use 'Annotations' rather than 'annotations' to propogate annotations to the stateful set pods. This change will allow the software to function as documented.

Previously, the rbd-mirror daemon was integrated into the `CephCluster`
CRD. This wasn't really practical since we would have to wait for the
whole orchestration to be done to actually set it up. The same goes for
any CR update. Let's say you want to change the number of daemons, Rook
would go through mons, mgrs and osds until it get to rbd-mirror.
This triggers an undesired full orchestration.
With its own CRD this component just gains a lot more flexibility.

In order to perform full automation of cephcluster deletion safely
and protect against catastrophic data loss, the end user must be
able to signify that they intend to irrecoverably delete the data
in their cluster. The cleanupPolicy field of the cluster spec is
intended to communicate this. This patch does not implement
automated deletion, but only creates the spec field and the
safety feature of halting orchestration other than deletion on a
cluster with a set cleanup policy value.

The pools had some legacy structs that translated between the
ceph.v1 types used by the CRDs and the internal implementation
of the pools. This simplifies the pool implementation by removing
the intermediate model and leaving us only with the ceph v1
pool types and no unnecessary translation.

The default pod anti-affinity for the mons that rook adds
automatically intended to be appended to any anti-affinity that
is specified in the cluster CR.
There is a bug in the ApplyToPodSpec() method that has long existed.
The issue is that when antiaffinity is appended, it will append not only
to the pod spec, but will modify the original placement spec. Thus, each
mon that is started will have one more antiaffinity clause than the previous mon.
This condition is rarely hit or noticed because it commonly is only hit
in the canary pods. Since these pods are immediately deleted,
there are no side effects of the duplicate affinity clauses. The place
where it becomes an issue is when the mons are backed by a PVC. In this case,
the mons do not have node affinity and will hit the code path that appends
to the antiaffinity and thus modifies the original antiaffinity.

Current YugabyteDB operator code creates Master and TServer pods without any resource requests/limits (specifically CPU and memory).
This causes the operator to run into soft/hard memory limit issue. The fix adds recommended resource requests and limits as defaults to the pods it creates.

Changed back the status.State to the exact way it was implemented on the ceph cluster. Added the States as a Constant and used that constant to call the status.State.

Most of the OLM UI currently uses status.Phase to show the status of the cluster and in some case it uses status.State to show the information.
Having them both for now and slowly deprecating the uses of status.state is the most appropriate way to do now.

The types that are used in CRDs across the storage providers do not need
to be in the v1alpha2 package. These should be declared stable in the v1
package. The only types remaining in the v1alpha2 package are the flex
volume types, which will never graduate since their replacement is the
csi driver.

As of Octopus, Ceph will prevent you from creating a pool with a
replica size of 1. Allowing such pool could lead to data loss, so enable
the new option: requireSafeReplicaSize: false if you are **ABSOLUTELY**
certain that is what you want.

We now support the addition of the PVC that acts as a metadata device
for a given OSD.
For this, you need to create a new `volumeClaimTemplates`, its name must
be "metadata" otherwise, Rook won't pick it up.
A template will look like this:
```
volumeClaimTemplates:
- metadata:
name: data
spec:
resources:
requests:
storage: 10Gi
# IMPORTANT: Change the storage class depending on your environment (e.g. local-storage, gp2)
storageClassName: gp2
volumeMode: Block
accessModes:
- ReadWriteOnce
- metadata:
name: metadata
spec:
resources:
requests:
storage: 6Gi
# IMPORTANT: Change the storage class depending on your environment (e.g. local-storage, gp2)
storageClassName: gp2
volumeMode: Block
accessModes:
- ReadWriteOnce
```
We now map block and block.db directly inside the container instead of
running ceph-volume activate. This is much cleaner.

the all annotations are merged with the component annotations.

calculation was crashing. Now we check for the case where

TuneSlowDeviceClass wasn't part of the StorageClassDeviceSet types but
only in VolumeSource which is not exposed in the CR spec so setting the
option would never work.
The only issue is that this apply to all volumeClaimTemplates
where each can point to a particular storage class...

In order for deployments managed outside the immediate scope of
the ClusterController to accurately detect the Ceph version of
their images, the ClusterController must store the detected
version. This change records the Ceph version as part of the
cluster status and modifies the crashcollector deployment to
consume this field to record its own Ceph version. This replaces
an earlier solution to this problem in which the Ceph version
for a given image to version mapping was stored in a global
in-memory map by the operator.

Multiple things:
1. We removed all the function/methods/tests that were used to
create and manage rook legacy OSDS as well as bringing support to
Bluestore OSD only.
It also fixes various go-lint issues in the respectives files.
2. use c-v inventory to detect available devices:
Now we rely on the 'ceph-volume inventory' command to tell us if a
device is available or not.
3. implement raw mode for osd on pvc
When an OSD will be bootstrap on a PVC, the new c-v raw mode will be
used. It consists of putting block, db and wal under the same device.
Here LVM is out of the picture and the raw device is used as is. The
implementation is backward compatible so existing OSD on PVC will LVM
will continue to operate.

A new CRD option `continueUpgradeAfterChecksEvenIfNotHealthy` is added.
When upgrading, Rook goes OSD by OSD and then waits for PGs to be clean
before proceeding to the next OSD. Currently, Rook waits for 5 hours but
there might be circumstances where PGs need more time to settle.
Thus setting `continueUpgradeAfterChecksEvenIfNotHealthy` to true will
pursue the upgrade process, even if PGs are not 100% active+clean.
