Travis Nielsen 34ca5d661b docs: refactor cluster crd doc for subtopics 
The cluster CR doc is extremely long and in need of refactoring.
Now we have four new sub-topics for a host-based cluster,
PVC-based cluster, stretch cluster, and an external cluster.
Each of those topics has its own description and examples.
The main cluster CR topic still contains the details about
all the possible cluster CR settings.

to help simplify the flow and make the different types of
upgrades more easily discoverable.

Travis Nielsen b6c2b99067 docs: factor out the csi custom image from upgrade doc 
Deploying a custom CSI image is more of a standalone topic
under the CSI configuration docs. There is no need for the
upgrade guide to have the details about CSI images.
