The ceph upgrade guide is factored out into a separate document
to help simplify the flow and make the different types of
upgrades more easily discoverable.

Deploying a custom CSI image is more of a standalone topic
under the CSI configuration docs. There is no need for the
upgrade guide to have the details about CSI images.

If prometheus is already installed in a cluster, it may not be configured
to watch for third-party service monitors such as for Rook.
To help with this configuration, a link is added to the doc for
enabling the third-party service monitors.

Support is removed for k8s for various limitations such as
priority classes not working and csi driver feature
incompleteness and missing snapshots. Documentation is updated
with the new min version of k8s 1.17 and also the tests are
updated to run on the min version of 1.17.

By default, we should set the priority class to one of the built-in
priority class names to ensure that pods critical to the storage
will be able to remain running when resources are low. Otherwise,
critical rook pods could be evicted and affect many other pods
that rely on the storage to continue functioning. The options have
been available in the CRs, but until now we have just not set the
defaults in the examples. Critical rook components are now set to
the priority class system-node-critical if they are generally pinned
to a node, and system-cluster-critical if they are critical to the storage.
Some pods such as the operator and crash collector do not have a
default priority class set in the examples since they don't affect
the data path.

Update the formatting so third-level headers are more visible,
sections are more consistent between the operator and cluster
chart docs, and a few other minor updates.

Ceph-CSI supports LUKS encryption for RBD volumes.
This commit adds support for the same by adding capability
of mounting required `rook-ceph-csi-kms-config` configmap
into csi-rbdplugin-provisioner and nodeplugin pods.
Required documentations, helm charts and examples yamls
are also updated.

The titles for the CRD documentation pages in the table
of contents were not consistent. The full name for the CRD
can be included in the full doc page, but in the TOC
we abbreviate the names for simplicity and readability.

finally, admission controller will be enabled default
without any script/manual step. But it still requires cert-manager
to be installed which I believe is already installed in clusters.
**Note**
Code doesn't return error it just logs the error since
we don't want to stop reconciling if the admission controller fails.
We can work on this once the admission controller is stable.

on several doc pages, causing any links using
{{ branchName }} to be broken on those pages.

The bluestore_fsck_quick_fix_on_mount fix was only needed when upgrading to
versions of Ceph older than v16.2.6. Everyone on pacific should now be
on v16.2.7, so we can remove this section from the v1.9 upgrade guide.
Also cleaning up a few other small issues in the guide.

to create rados namespace for a given
ceph block pool. Typically the name of the pool
is the name of the blockpool created by rook.

various part of current code. With PR the value is always `us-east-1` so
that it will work RGW server without any issues.

The prometheus rules had been previously created if the cephcluster CR
setting monitoring.enabled was set to true. The rules were not customizable
and therefore not flexible enough. Now the rules are installed by the helm
chart. To customize the rules, a post-processor can be applied to the helm
chart.

available in the cluster CR to that ceph will be automatically
