Add a more involved multus validation test to the Rook binary. Because
this is intended to be end-user runnable, make sure operator-only
commands are hidden.
Build this into the rook binary instead of creating a separate binary
for ease, and because any binary built with the kube api becomes 40+
megabytes. We save quite a bit of space by including this in the Rook
binary, which is good for keeping container layers as small as possible.

With the new mgr HA implementation (based on readiness probe) we
don't need the mgr sidecar (live-watch) anymore. This commit is

Environment variables are not recommended for secrets in pods since
they can be easily leaked if the environemnt variables are logged.
By mounting the mon secret as a file, the mgr and osd prepare pods
can read the mon secret from a file for better security.

The flex driver has been fully deprecated and thus removed from Rook.
Before upgrading to v1.8, users will need to convert existing flex volumes
from flex to csi volumes.

Our admission webhooks will now run as part of the Operator container
and not an additional deployment. This has the advantage of consuming
fewer resources in the cluster and not having to manage affinities and
tolerations. This only drawback is that the Secret containing the
certificates is not mounted anymore and the content needs to be written
inside the Operator. This is not practical since we also need to watch
for the Secret content to change. Meaning that the certificates have
been renewed and the webhook server needs to use them.
A new approach is on its way to hopefully simplify this last issue and
implement a watcher for the Secret.
In the meantime, users need to use the cert-manager or renew
certificates manually. Additionally, they must update the
ValidatingWebhookConfiguration object with the new CA bundle.

The mgr daemon may be failed over by ceph if the active mgr is not
responding and the standby mgr is available. If the active mgr changes
the services for the dashboard and metrics will be updated with a
label selector for the new active mgr. The services cannot direct
traffic to the standby mgr or else they will be incorrectly redirected
to the active mgr.

To provide more context for executing commands in a ceph cluster,
the full clusterInfo is now passed to the ceph execution commands.
All information about the cluster will now be available throughout
all the areas of the operator. The namespace, ceph credentials,
mon endpoints, and other info is a core part of that cluster info.
Arguments passed through the controllers are also simplified for
mons, mgr, osds, and other daemons where the parameters had
become too complex.

The ceph config and client packages are conceptually the same.
To avoid circular dependencies in some cases, we simplify by combining
the packages into the client package.

The operator should only connect to ceph with a single set of creds.
In a converged cluster this will be the admin creds and in an external
cluster it will be lower-privileged creds. Independent clusters were
implemented with a separate set of creds. To simplify the code these
are now merged to a single set.

Multiple things:
1. We removed all the function/methods/tests that were used to
create and manage rook legacy OSDS as well as bringing support to
Bluestore OSD only.
It also fixes various go-lint issues in the respectives files.
2. use c-v inventory to detect available devices:
Now we rely on the 'ceph-volume inventory' command to tell us if a
device is available or not.
3. implement raw mode for osd on pvc
When an OSD will be bootstrap on a PVC, the new c-v raw mode will be
used. It consists of putting block, db and wal under the same device.
Here LVM is out of the picture and the raw device is used as is. The
implementation is backward compatible so existing OSD on PVC will LVM
will continue to operate.

- Added code to support StorageClassDeviceSet spec provided in the cluster-on-pvc.yaml
- The code reads the StorageClassDeviceSet spec and creates pvc based on the ‘count’ field for each device set.
- OSD prepare job is started for each PVC which activates the ceph-volume on each PVC
- Finally OSD is started on each of the PVC device.

Refactor the Ceph NFS (Ganesha) daemon to use the new config pattern.
Use of a custom Rook binary to run NFS has been removed in favor of
instead running the required DBUS process as a sidecar. The effect here
is that the NFS Ganesha container itself may fail and restart a few
times before the DBUS sidecar is running.

This change is meant to allow running operators locally on a developer machine.
The idea is to allow faster development cycles by reducing the time and complexity of building -> deploying -> debugging on cluster.
For operators that rely only on kubernetes API this works easily - see cockroachdb and minio examples in development-flow doc.
The change includes:
- rook.NewContext() - Refactored to remove repeating initialization code that was copy-pasted in most of the operators in order to create the clusterd.Context and the Clientsets. Also it detects the mode of working in-cluster vs external and sets up the external mode with standard user config (~/.kube/config) and a job executor.
- rook.GetOperatorImage() - Refactor this repeating code in many operators to detect the operator pod image. Also added a global flag --operator-image that developers can use to override this when running locally.
- rook.TerminateOnError() - Added a convenient function.
The basic idea here is to replace the release name in the ClusterInfo
object with the fine grained version information queried at runtime.
This patch also removes the Name field from the cluster spec, which was
also runtime determined and was redundant with ClusterInfo relase name.

Configure the Ceph rgw daemon completely from the operator a la the
recent changes to the Ceph mon, mgr, and mds operators.
Create the rgw deployment or daemonset first, and then create the
keyring secret for the object store with its owner reference as the
corresponding deployment or daemonset. When the replication controller
is deleted, the secret is also deleted.
The RGW's mime.types file is now stored in a configmap with a different
file created for each object store. This is primarily just a means to
get the mime.types file into the rgw pod, but the added benefit is that
the administrator can modify the configmap, which could reduce
susceptibility to file type execution vulnerabilities (worst case).

Configure the Ceph mds daemon completely from the operator a la the
recent changes to the Ceph mon and mgr operators.
Create the mds deployments first and then
create the keyring secrets for them with their owner reference as the
corresponding deployment. This will mean that the secrets do not need to
be micromanaged. When the deployment is deleted, the secret is also
deleted. This has not been necessary for the mons or the manager since
the mons share a keyring with a lifespan of the cluster, as does the
mgr, which currently has single-mgr support only.

This commit introduces the necessary changes to support the new
messenger feature coming with Ceph Nautilus (currently in development).
What changes? Now the monitor listens on two port:
* old 6789 for messenger v1, which will help us support older client
(e,g: krbd)
* new 3300 for messengers v2, which brings new improvement in the
messaging layer. This new transport layer brings numerous advantages
such as encryption improvement, speed improvement, pluggable nature to
support different network stack than TCP and many more.
We still have one Service IP, however it has 2 ports, see:
```
NAME TYPE CLUSTER-IP EXTERNAL-IP PORT(S) AGE
rook-ceph-mon-a ClusterIP 10.106.217.160 <none> 3300/TCP,6789/TCP 4h
rook-ceph-mon-b ClusterIP 10.99.36.175 <none> 3300/TCP,6789/TCP 4h
rook-ceph-mon-c ClusterIP 10.108.220.74 <none> 3300/TCP,6789/TCP 4h
````
The `ceph.conf` has changed and we don't force the port when using an IP
address (public addr etc). Ceph, depending on its version will naturally
start the monitors on their right port, 6789.
A new --ceph-version-name CLI argument has been added to the Rook binary
so that when the pod starts it passes the ceph version name and the
configuration of the ceph.conf, as well as daemon startup flags, happen
properly.
Given that the Rook Operator remembers the port of all the monitors it
deployed (through Pod definition), this change is not an issue and will
maintain backward compatibility.
Note that to test this you must build rook with dev container image,
which contains the dev Nautilus version. So you should do something
like:
`make -j4 BASEIMAGE='ceph/daemon-base:latest-master' IMAGES='ceph' build`

Configure the Ceph mgr daemon completely from the operator a la the
recent changes to the Ceph mon operator.
The pod spec for the mgr changed quite a bit, and instead of updating
the unit tests of questionable use, some additional unit test tools
applicable to any Ceph daemon have been added and used with the mgr. The
mgrs unit tests should now be more useful and get in the way of devs
less, and they can be used by other daemons later.

Make the Rook config-init unnecessary for mons, and remove that init
container. Perform all mon configuration steps in the operator, and set
up the mon pods and k8s environment such that only Ceph containers are
needed for running mons.
This should help streamline changes to the mons, as there will be no
need to change the `daemon/mon` code or `cmd/rook/ceph` code with mon
changes in the future.
This work starts to lay the groundwork for supporting the
`design/ceph-config-updates.md` design.
Notable new bits:
Create a keyring secret store helper for storing dameon keyrings, and
use it to store the mon keyring. Mon pods mount the keyring into a
k8s secret-backed volume.
Create a configmap store for the Ceph config file which can be mounted
into pods/containers directly to /etc/ceph/ceph.conf. Also store
individual mon_host and mon_initial_members values which can be mapped
into pods as environment variables and used in Ceph commandline flags,
enabling the mon pods to have the most up-to-date information about the
mon cluster when restarting and without need for operator intervention.

Create a cephconfig module in Ceph's daemon pkg source, and refactor the
config and keyring generation that exists in the mon package into the
new cephconfig package. The config/keyring generation code is used by
most all daemons and not just mon, so a new package is a more
appropriate place for this.
