Travis Nielsen 92e7f12451 cephcsi: add upgrade documentation for csi driver ...

If using ceph-fuse or rbd-nbd the application pods will need
to be restarted during upgrade. This adds a description of
the steps necessary to ensure the applications remain healthy
during upgrade.
