Anmol Sachan caf79f971f ceph: Fixed CephMonHighNumberOfLeaderChanges alert ...

The alert was showing wrong POD and hostname in the
alert description. Fixed the query for the same by
adding the right labels from ceph_mon_metadata query
to the original query.
