Update both documents and `upgrade-from-v1.2*.yaml` files for Rook v1.3
release.
Of note:
- note when the upgrade to CSI driver v2.0 should be updated
- note that CSI image envs can be removed to have Rook use defaults
- update Ceph version upgrade section
- update supported Ceph version info
- update CRDs before upgrade b/c new 'status' fields
- remove special CSI update info left from v1.2
