Unify naming for Qt Online Installer and Qt Maintenance Tool. Ass macros for theses
as well.

qmake files were still referenced.
Do not focus on building Qt Creator, we ship a development package
nowadays which is preferred.

Source and configuration files for each manual are now located in a
separate subdirectory, with common configuration files in doc/config.

Edit the config files accordingly.
