Move Status enum out of QmlDebugClient and drop "QmlDebug"
prefix for the different 'Reference' structs. Allows to
avoid 'using namespace in QmlDebug' in header files.

Merge QmlJSInspector plugin into the debugger. Also merge the
extra Inspector window with the Locals & Watchers: It now shows
the QML object tree in the running state.

That's a reminiscent of the times when qmljs was just a copy
of the classes in Qt.
Rather include the files individually.

Rename the QmlJSDebugClient lib to QmlDebug (shorter names, easier differentiation with the shared/qmljsdebug lib).
Also rename
- QDeclarativeDebug* classes to QmlDebug*
- QDeclarativeOutputParser class to QmlOutputParser
To clarify the features,
- Debugger::QmlDebuggerClient is now Debugger::BaseQmlDebuggerClient
- QmlEngineDebugClient is now BaseEngineDebugClient
- QmlDebuggerClient is now QmlEngineDebugClient
- QDeclarativeEngineDebugClient is now DeclarativeEngineDebugClient

Actually wait for the 'Waiting for debugger on port ' ... message
to appear in the application output before connecting, and also
use the port specified there.

retrieve and emit the debug output.

V8DebugService uses a different protocol. This fix now uses the
new protocol. It also optimizes the number of debug requests made
to retrieve debug information.

Expose the current debugger engine (with it's start parameters) in
Debugger::QmlAdapter, and use it to initialize the FileInProjectFinder
in QmlJSInspector.

The ScriptConsole is renamed to QmlJSScriptConsole. The console can be used
to evaluate expressions during a QML application debug session.
Changes:
* Console can be used even when application is not on a debug break
* The object context is the currently selected object in Inspector
* ContextMenu has a Clear option
* Multiple line expressions are supported
* Traverse Command History using up and down arrow keys

Use "QML Debugger" as in qmlengine.cpp . Also don't print a warning
if the status is 'unavailable' as this is the default first state
in ',block' mode.

Sends connect command after all initial commands have been sent.
This ensures that the initialization is complete before the start
of the debugging session.

Not fully working over USB yet, there are some handshaking issues that need to be addressed. WLAN might be improved due to the -qmljsdebugger fixes, but not tested.
When closing the app via the WM the status of the services change
to 'not available'. We used to log this as an error to the debugger
log, causing the log to 'magically' pop up.
