Turns out we don't even need two-phase initialization,
by transparently postponing registration until we get
a non-empty file name, either at constuction, or at
file name change times.

There was only a single user (TextEditor::DocumentMarker) which
in turn was only used in a single place (BaseTextDocument), so
merge the ITextMarkable into BaseTextDocument.

This requires moving the activateEditor call into openEditorWithContents.
Remove that line elsewhere when editors are constructed. Keep it when
reusing an existing editor though.

And where using editors is necessary, take *all* editors into account,
not a random set of what previously was called "original" editors (when
using splits).

Ensure that it is updated as the lines are moved. That can be later used

ITextMark is not abstract anymore and has an icon and a priority.
This means separate breakpoint and location marker classes that
are only "plain" marks with icons and priorities are not needed.
BaseTextMark directly inherits from ITextMark, instead of owning
an ITextMark derived InternalMark.
Also, there is now ITextMark::paint() to make it a bit more flexible
then icon()[->paint()]
