13675cf6ed Cleanup the C++ front-end. 
Removed CPLUSPLUS_BEGIN/END_NAMESPACE & co and made it possible to compile the parser with CPLUSPLUS_WITHOUT_QT.

ecc2b25ddc Extended the AST with references to more tokens 
Mainly comma tokens. Referencing all tokens is necessary to be able to
implement pretty-printing based on the AST while maintaining user
formatting.
