Getting the #include directives ready for Qt5. This includes the
new-project wizards.

* Don't add huge Nokia headers to the generated files.
* Use uppercase header guards based on plugin and example object names.
* Moved qdeclarative.h include to .cpp file since it's not needed in the
header.
* Use user-defined source and header suffixes.
* Lowercase file names.
