If the user has enabled clangd (default is off), we start up one instance
per project when it is opened/changed (including build config switches),
and trigger background indexing.
So far, the index is used to provide results for locators and "Find
Usages".
Per-document functionality such as semantic highlighting and completion
is still provided by libclang.

As ClangDiagnosticConfig() has tidy and clazy enabled by default now,
the ClangTools' importDiagnosticConfigsFromCodeModel() takes diagnostic
configs on start up away from CppCodeModelSettings.
Ensure that built-in configs do not have tidy/clazy enabled by default.
Also, when importing, look at the actually enabled checks.

Add a separate pool of custom diagnostic configs for the
ClangTools plugin. That is, the diagnostic configs in
Menu: Tools > C++ > Code Model
are not shared anymore with the configs at
Menu: Tools > Analyzer > ClangTools
On plugin initialization of ClangTools, move tidy/clazy related configs
to ClangTools.

1. Create some Qt Console Application from the wizard
2. Open Tools > Options > C++ > Code Model
2.1 Click "Manage..." and create a custom configuration "custom" and
finish with OK.
2.2 Set "custom" as diagnostic config.
3. Analyze > "Clang-Tidy and Clazy..."
3.1 Use "Custom Settings"
3.2 Click "Manage..." and remove "custom" ==> Crash
The crash happened because the code model used an invalid diagnostic
config id. The invalid id came from the settings, which were not
correctly upated.

The diagnostic id might not match any diagnostic configuration in the
settings as Qt Creator prior version 4.8 could have written it
inconsistently. We need to correct this case.

List of checks does not give enough flexibility to
select/unselect specific checks. The tree fixes that.
Also remove Clang-Tidy checks line edit because it is
now integrated into the tree mode as an alternative way
of providing checks by pressing "Plain text edit" button.
'cpptools_clangtidychecks.h' is generated using python
script 'generateClangTidyChecks.py' and clang-tidy
from our LLVM/Clang 6.0 build.

A warning configuration is a list of command line (warning) options for
libclang. Three non-editable built-in configurations are provided by
default. The user can copy a configuration to customize it.
This is still a global setting and it changes take effect after
re-opening a document. Both issues will be addressed in follow-up
changes.

The model-manager now supports multiple code models for semantic
highlighting and code completion, and will choose one based on the
mime-type of the editor.
The settings page is currently disabled. It will get enabled when a
second plug-in lands that has a ModelManagerSupport class.
