Always exclude clang include paths which do not come
with libclang used by code model.
This solves the issue with GCC that uses system clang
include folder on macOS.

The argument parsing has some considerable overhead. We try to avoid that
with merging all content together in one file.

We used the same code in different but don't shared it. From a bug fixing
perspective it's actually not that smart.
