Immediate proposals are displayed for asynchronous providers and fragile
proposals.
The idea is to show already available results immediately. The proposal
calculated in the process runner can then replace the immediate
proposal.

If two files from different (sub-)projects include the same header file,
and the defined macros differ for both files, the header file will be
parsed with only the appropriate macros for the including file.

This is step 1 of 2 for merging the various provider factories into a
single class. Merging has the advantage that selecting based on editor
(content) mime-type only has to select one class, instead of re-doing
the selection for each class separately.

This change addes support for class, enum definition inside blocks({}) or
functions, e.g.:

It fixes:
* code completion
* highlighting
* follow symbol
* marking
* find usages
It fixes also problem with namespace aliases inside blocks or functions.
This change can have also impact on performance(there are additional processing)

Caused by a dangling pointer of a template instantiation which had been
cloned into the wrong control. The fix is to remove that control and
refer to the control of the bindings (which is the correct one).

Introduces ITextEditorDocument. This is part of a more general "use
documents instead of editors whereever possible". It will allow to move
to e.g. ITextEditor::openedTextDocumentContents() instead of
ITextEditor::openedTextEditorsContents().

Steps to reproduce the crash:
1. Open some long file, e.g. botan.cpp
2. Trigger completion and close editor immediately
The IDocument pointer was only used to reference the file name. Instead
of passing an IDocument* to the IAssistInterface, pass the file name.

Reduce the padding in some often-used structs for 64bit machines.
32bit machines should also profit from most changes, but to a

This was the indirect cause for "random" freezes when using
code-completion. The direct cause was a while loop counting down to 0,
but starting with a negative value. Both are fixed now.
