If the user has enabled clangd (default is off), we start up one instance
per project when it is opened/changed (including build config switches),
and trigger background indexing.
So far, the index is used to provide results for locators and "Find
Usages".
Per-document functionality such as semantic highlighting and completion
is still provided by libclang.

Add a separate pool of custom diagnostic configs for the
ClangTools plugin. That is, the diagnostic configs in
Menu: Tools > C++ > Code Model
are not shared anymore with the configs at
Menu: Tools > Analyzer > ClangTools
On plugin initialization of ClangTools, move tidy/clazy related configs
to ClangTools.

QComboBox, QLabel and QPushButton are always used
together. Therefore it makes sense to put them in one
custom widget.

Now selection is only consists of combobox and
a "Manage" button to diagnostic configurations.
Diagnostic configurations are moved to the modal dialog
which is shown by clicking the mentioned "Manage" button.

top is perceived as the main widget:
* Remove the "Configuration to use:" label so the combo box gets more
horizontal space and thus dominance. Also, rename the group box
header/label from "Clang Diagnostic Warnigns" to "Clang Diagnostic
Configuration".
* Move the command line text edit below the combo box into its own tab
page to clarify the relation to the tidy/clazy tabs.

Add UI controls to change settings and apply them
together with warnings and command line options.
Current settings are not very flexible but should be
easy to test and use without reading tidy/clazy help.

This introduces a new option in Options > C++ > "Code Model" to

A warning configuration is a list of command line (warning) options for
libclang. Three non-editable built-in configurations are provided by
default. The user can copy a configuration to customize it.
This is still a global setting and it changes take effect after
re-opening a document. Both issues will be addressed in follow-up
changes.

The model-manager now supports multiple code models for semantic
highlighting and code completion, and will choose one based on the
mime-type of the editor.
The settings page is currently disabled. It will get enabled when a
second plug-in lands that has a ModelManagerSupport class.
