If a document has a file name associated with it then the prefix is
composed from path components (subdirectories), starting from the one
where the file is located and going up the parents until the resulting
name becomes unique among other open documents.
If a document doesn't have an associated file name, then a sequential
number (starting from 1) is appended to the display name of the
document.
This feature is useful when working with big projects that have lots
of idendical file names across different subdirectories (e.g.
Makefile.in, main.cpp, etc.) that need to be edited at the same
time. It allows to easily recognize such a file when switching
between documents in the editor, w/o the need to place the
mouse pointer over the name entry to get its full path.

Rename VcsCommand to VcsCommandTag, Command into VcsCommand

If there are other editors on the document visible in some split, it
should not close the document, but only the editor.

The VCS plugins keep a reference to the submit editor, so it can
explicitly be closed, instead of relying on the correct currentEditor in
the editor manager.

* Use paths/default-push and paths/default Urls as appropriate
* Read the settings from the project location except when the
currently open file is not below that project directory.
This makes things work for QTCREATORBUG-10261 without breaking
.hgsubs (which are required to be children of the top-level
repository.

This change
* Removes IMode::type, ModeManager::activateModeType, and
IEditor::preferredModeType, and adds IEditor::isDesignModePreferred
instead
* Adapts the mode switching code in EditorManager to handle multiple
windows, for example switching to edit mode should only happen if
the editor/view is in the main window. Otherwise the editor window
should be raised and focused
* Renames EditorManager::NoActivate --> DoNotChangeCurrentEditor
* Reverts the EditorManager::ModeSwitch logic to switch mode or
make the current editor visible by default, introducing
DoNotMakeVisible flag instead
* Fixes a few instances where EditorManager::ModeSwitch should have been
used
One non-trivial problem left: If you open a .ui file and switch to an
external editor window, edit mode is activated, because the current
editor no longer is a .ui file, which means that the design mode gets
deactivated.

It was not possible to simultaneously open two commit editors for
different version control systems, also there was no reason to scan all
open editors for the submit editor, since the plugins can just remember
the editor that they opened.
