This eliminates a bunch of list->set->list conversions. Especially the
ProjectInfo::appendProjectPart takes lots of time converting for every
part added.

Replace the not thread safe Project::files() with the thread safe
CppModelManager::projectPart() to find out whether a file belongs to a
project or not.

the line and column numbers correctly with respect to unicode code
points.

CppTools plugin. Rename them and do some other small namespace
related coding style fixups.

while i'm at messing with include statements, also re-order the include
blocks according to policy and sort them within bigger blocks.
