This introduces an API change for the project managers. Those are not
expected to call updateSourceFiles() anymore.

For the quick fix AddIncludeForUndefinedIdentifier, if no class is found
via the locator, check the "Qt include paths" for a header file with the
same name as the class name.

model information from ProjectExplorer::ToolChain.
This change provides unified way.

Add state logic to CppCodeModelManagerInterface, making it aware whether
an indexer is running, protect the update methods from another
invocation while running. Add changed signals to IVersionControl and
VCSManager and wire them to the update methods. Add a menu action for
manually updating.

Make debugger tooltip API more general, have the engines check the
correct file type, figure out the expression and context from the text
editor.
Put common functionality in watchutils.cpp. In the CDB engine, check
whether a tooltip expression is a known variable within the stack frame
context. If so, retrieve via symbol group or dumpers. Cache by function
and expression.

This adds a way for plugins to tell the cppmodel that for certain files,
the actual contents come from it, instead of reading from the disk.
(For texteditors the cppmodel does that automatically.)
