QmlProfilerDataModel does it the same way and we don't pollute the
namespace as much like this.

They should both look up the source details when loading data
and they shouldn't do that again on gotoSourceLocation.

The model manager should only set its state to 'Done' if all models are
actually done. When that is the case it can safely emit dataAvailable,
too, freeing us of the need to apply a heuristic to the progress
percentage. In order to have a unified interface to the completion of
model processing an abstract base class for QML and V8 models is
introduced.

In the case that the application did not generate any event
after running the profiler, still show an empty trace.
Otherwise the user will think that the profiler didn't work.

Rename the QmlJSDebugClient lib to QmlDebug (shorter names, easier differentiation with the shared/qmljsdebug lib).
Also rename
- QDeclarativeDebug* classes to QmlDebug*
- QDeclarativeOutputParser class to QmlOutputParser
To clarify the features,
- Debugger::QmlDebuggerClient is now Debugger::BaseQmlDebuggerClient
- QmlEngineDebugClient is now BaseEngineDebugClient
- QmlDebuggerClient is now QmlEngineDebugClient
- QDeclarativeEngineDebugClient is now DeclarativeEngineDebugClient

The code of the qmlprofiler client has become a bit too complex,
this patch reorganizes the modules in a more sensible way,
having the modules communicate with each other through a
state machine instead of the excess of signals and slots
from before.
