Always include add_reference in iterator_adaptor.
It's only included for certain configurations but is always used. This didn't matter in the past because it was transitively included by is_convertible. But it isn't now on Visual C++ because its itsintrinsic version is being used, which doesn't require add_reference. [SVN r69855]
