Hervé Brönnimann 729bb65a2d Removed minmax.hpp from boost (leave Eric Niebler's former file)

and move it to subdir boost/algorithm/
Also incorporate boost::ref into boost/algorithm/minmax.hpp  to avoid
copy constructors when params passed by reference.
