3c0df453c3 Removed code-breaking change from boost/detail/iterator.hpp and corresponding workarounds from tests. 
Added permutation_iterator_test to the suite after fixing it up -- it
was riddled with bugs!

e0f121df34 Moved to much cleaner system of using BOOST_TT_BROKEN_COMPILER_SPEC for handling vc6/7 deficiencies with iterator_traits. 
Fixed a bug in iterator_facade which was causing incomplete types to
be passed through is_convertible.

Reinstated libs/utility/iterator_traits_test.cpp

61cc24992c Always have a definition for each traits member, even if it can't be properly deduced. These will be incomplete types in some cases (undefined<void>), but it helps suppress MSVC errors elsewhere
