Daniel James bfb19140f1 Merge documentation fixes.

* Use `doc/src/*.css` instead of `doc/html/*.css`.
* Remove wiki and people directories.
* Some documentation fixes.
* Left out `minimal.css` changes and boostbook changes because of clashes.

Merged from trunk: for visual studio 8 and visual studio 9, reverted to using sizeof in 'native' typeof operator as the use of typeid caused compilation error in visual studio source code analysis.
