The topics section of the ceph common problems troubleshooting guide
contained many subsection entries for symptoms, investigation and
solution.
Apart from not having working links,
these subsection entries don't add any real value:
They are well visible within the overarching section
and in the toc on the right side.
This change cleans the toc by removing these unnecessary subsection
entries.
This also reduces the number of nonexisting anchor messages
of make docs-build from 50 to 18.

the ceph exporter port was hardcoded to 9926, preventing
co-location of exporters from different ceph clusters on
the same node. add a port field to cephexporterspec so
users can override it via spec.monitoring.exporter.port.

when cluster is deployed in namespace other than rook-ceph,
csi-operator.yaml should also follow similar approach on how
rook operator.yaml and other yaml are updated to deploy in
alternate namespace using the comment `# namesapce: operator`.

volume node loss case, let's remove rbd specific
wording from the node loss doc in rook.

The setting for concurrent clusters is now available in the
helm chart, instead of only as an env var on the operator
deployment. reconcileConcurrentClusters will be applied to the
env var ROOK_RECONCILE_CONCURRENT_CLUSTERS.

Read affinity in Ceph v20.2.0 can cause corruption.
Disable this feature for this version of Ceph to
avoid hitting any corruption in this case. When the next
patch release is out for tentacle, this issue will be
resolved and when upgrading to it, read affinity would
automatically be re-enabled if needed.

that must be enabled explicitly with a setting on the pool.
Add this setting to the docs and examples.

The activeStandby property was being ignored when deciding
how many mds daemons to start. 2x the active count of mds
daemons were always being created. If the standby is not
desired, the standby can be removed, and it was unexpected
that it was not removed.

Document how to migrate an application with a dynamic
PV rather than a static PV, since static PVs have limitations
such as not supporting snapshots and clones.

Noticed two small typos when reading the documentation on object stores.
I also noticed that the lack of a whitespace in the creating a user
example affects the syntax highlighting.
