9a4284d666 Clang: Use CppHoverHandler for diagnostic tooltips 
We used to call QTextCharFormat::setToolTip from the ExtraSelection to
install the diagnostic tooltip. Since this allows to set only text
tooltips and we would like to introduce a custom tooltip widget for
diagnostics, make use of CppHoverHandler, which is more flexible.

6ed775f56a Fix wrong context help if tool tip was not shown for focus widget 
The tool tip never has focus, so it cannot become the IContext that is
checked for context help. So, integrate the help id into Utils::ToolTip
and check the tool tip first when checking for context help.
As a side effect the [F1] button and help id for the tool tip is now also
available for use outside of the text editors.

beef480783 TextEditor: Simplify HoverHandler handling 
The editor factories are already a central place to associate
hover handlers with editors, no need to retrieve them later from
the object pool again. This also allows for easy handling of
more than one active handler per editor.

1d5091e48f Do not use deprecated Qt functionality. 
Replace all* remaining deprecated Qt 4 functions with
their Qt 5 counterparts. This means we no longer need to
define the QT_DISABLE_DEPRECATED_BEFORE macro.
This patch is relatively small because most source-compatible
changes of this kind have been done before.

* The one exception is the QmlDesigner, which uses QWeakPointer
in a deprecated way all over the place.

0f2a0d9358 C++: cache results of linksForIdentifier in HelpItem. 
The method linksForIdentifier was called at least thrice when generating
a single valid tooltip in the C++ editor. Now the cached, and the cache
can be "initialised" during construction. This reduces the time spent
for creating a tooltip by 30%.

7923032022 fix include style relating to cplusplus libraries 
... and adjust INCLUDEPATH accordingly.

while i'm at messing with include statements, also re-order the include
blocks according to policy and sort them within bigger blocks.

922e8e7866 Help: All candidates (considering name qualification) are already in the list. 
The C++ hover handler should just iterate over the list of candidates, since
it was created by the element evaluator considering all possible name qualifications
and in the right order.
